Systematic (IUPAC) name :
6-(4-dimethylamino-3-hydroxy- 6-methyl-tetrahydropyran-2-yl) oxy-14-ethyl-12,13-dihydroxy- 4-(5-hydroxy-4-methoxy-4, 6- dimethyl-tetrahydropyran-2-yl) oxy-7-methoxy-3,5,7,9,11, 13-hexamethyl-1- oxacyclotetradecane-2,10-dione
Clarithromycin is a semi-synthetic derivative of erythromycin. Clarithromycin differs from erythromycin in it ís methyl substitution at the number six position of the macrolide ring.
Its Chemical name is 6-0-methylerythromycin. Its Molecular formula is C38H69NO13, and the molecular weight is 747.96 .
clarythromycin is a white to off-white crystalline powder. It is soluble in acetone, slightly soluble in methanol, ethanol, and acetonitrile, and practically insoluble in water . 

Pharmacology and Medicinal use : 
Clarythromycin, is a macrolide antibiotic similar to Erythromycin and Azithromycin, it is effective against Mycobacterium avium complex(MAC) and is used for treatment of Helicobacter pylori associated peptic ulcer disease, community-acquired pneumonia, and chronic bronchitis. 
Clarythromycin is also used to treat respiratory tract as well as uncomplicated skin and skin structure infections, sexually transmitted diseases, otitis media, and AIDS-related infections. It is effective against a wide variety of bacteria such as Haemophilus influenzae, Streptococcus pneumoniae, Mycoplasma pneumoniae, Staphylococcus aureus, mycobacterium avium, etc. 

Pharmacokinetics : 
Clarithromycin is absorbed rapidly from the gastrointestinal tract, distributed into body tissue and fluids, and crosses the placental barrier. The drug is principally excreted via the liver and kidneys, therefore, in hepatic and renal impairment it should be used cautiously. Unlike erythromycin, clarithromycin is acid-stable and can therefore be taken orally without being protected from gastric acids. It is readily absorbed, and diffused into most tissues and phagocytes. Due to the high concentration in phagocytes, clarithromycin is actively transported to the site of infection. During active phagocytosis, high concentrations of clarithromycin are released. 
The concentration of clarithromycin in the tissues can be over 10 times higher than in plasma. Highest concentrations were found in liver and lung tissue. 

Mode of Action : 

Clarithromycin prevents bacteria growth by interfering with their protein synthesis. Clarithromycin binds to the subunit 50S of the bacterial ribosome, and thus inhibits the translocation of peptides. Clarithromycin has similar antimicrobial spectrum as erythromycin, but it is more effective against certain gram-negative bacteria.
